Registration Information

EARLY BIRD DISCOUNT BEFORE March 7, 2018: $180.00
$200.00 ($185.00 + $15.00 onsite fee)
Groups of 5 or more is $170.00 p/p (one payment transaction per group. No add-ons after payment process)

Registration includes: program, syllabus, continental breakfast, lunch, and CME accreditation.
Non-physicians receive a certificate of attendance to be submitted to their own credentialing board.

Cancellation Policy: Cancellation request must be in writing and received by March 7, 2018. A $20.00 fee will apply. No cancellations accepted after March 7, 2018. However, registration may be transferred to another person.

PAYMENT METHOD: Online now (with your credit card), or mail in your check (see below). Online registration only accepts credit cards payments.
